Social Services
Social workers in outpatient settings deliver services that assist patients & providers. The key functions and tasks performed by the
social workers include:
- Assistance with care coordination involving home health
& other referrals
- Assisting patients & families with locating financial
and/or medication assistance
- Assistance with referrals to specialized community services
such as medical transportation, senior
support services, food assistance, housing issues, etc.
- Educating patients on how to apply for federal, state & local assistance programs
- Educating & assisting with Advance
Directives & Living Will forms
- Assessment & intervention in suspected abuse or neglect situations
- Assessment for need of emergency mental health
interventions
